A New Model of PT: Scale without Headaches

Eventually, every business hits the moment of deciding how they will scale. What that typically means is acquiring more clients and, in turn, acquiring more staff. At least, that’s how it works in service-based businesses that coaches and rehab professionals work in. 

But what if there was an option that allowed you to generate more with LESS of these things? 

Why does nobody talk about scaling via PROCESS? AKA become efficient AF.

Now this will require you to challenge some beliefs and think outside the box for new ways to do things, but many of the concepts should feel familiar.

Every business tries to figure out ways to maximize revenue. And in traditional service based businesses like PT, you generally see support staff to help manage that volume.

For example, many traditional physical therapy clinics bring on PT techs to manage the exercise and client flow throughout the day of patients. 

This is an example of combining staff and process efficiency. This model took the portions of treatment that did not require a licensed PT and replaced them with a staff member who could be brought on for a fraction of the PT wage.

In the fitness industry, you’ve seen this implemented with a group class model. The change in process and operations allows for one coach to manage many clients. 

Now when we look at high-value coaching and rehab professional treatment processes, they are typically marketed as 1:1 (meaning the person providing the service is directly involved in the management of the entire client session) and often sold in units of time, typically 1 hour in length. 

Ironically, the thing that broke us out of the traditional rehab model also ended up being one of the biggest scale constraints in the existing cash-based model. 

Can we admit that we just traded a big problem for a smaller one? At least that’s how I felt…

That was until I started leveraging my time with a virtual offer. What I now call a Prescriptive Exercise Program (PEP for short). 

This allowed me to get solutions and outcomes for my clients WITHOUT tying me to a session. I finally felt like I was being valued for something other than time… and for the first time in my career, I felt like I had the freedom I was trying to achieve. 

So how does it work?

The PEP must look and feel different than a home exercise program and it certainly was not an all encompassing fitness program…

This was how I structured it: 

As the provider, I positioned the PEP as an experience that extends beyond the facility walls

I’d write the program, and my clients would  implement it alongside their standard strength and conditioning work, in their own facilities or gyms. 

One day per week, I’d spend a few allocated hours writing the weekly programs for your clients and interpreting their feedback so I could make the appropriate adjustments to the next batch of work.

From a financial standpoint, it changed a lot about how I viewed my business. 

Let’s say you currently charge $200/hour for in-person services and only see one client at a time. That means the earning potential for that hour is $200. 

Now, let’s say you have five clients on a monthly recurring Prescriptive Exercise Program at $300/client per month. Now I could manage each client in about 45 min - 1 hour per month total…

When it takes you 45 min per month = $400/hour (double your effective hourly rate)

Even if it took you 1 hour per month… $1,500 in revenue divided by 5 = $300/hour… (1.5x your effective hourly rate)

Don’t believe this is possible? At my peak of managing clients remotely, I had a 56-client roster and used 8 hours per week to manage them. 

That's about seven clients per hour of management time. This efficiency is around $488/hour.
